Ok, here are some instructions that will take the 11 MP4 files and repackage them into an MKV file with correct chapters.

Firstly, you will need to combine the 11 MP4 files into a single MP4 file.
Download MP4 Joiner from here: http://www.mp4joiner.org/en/
This program is self explanatory - just click the "Add Video" button, select the files you want to join (in order) and press the "Join" button. This will create a new MP4 file - no re-encoding will occur so it should only take a few minutes. For some reason it takes a long time on my PC, so don't be concerned if it takes longer.

Once you have got your single MP4 file, you can convert it into an MKV and at the same time add chapter points by using a program called MKVToolnix. You can download it from here:

In order to add chapter points, you will need to create an XML file. The structure is below. This particular chapter file only contains 2 chapter points, but you can download the full one from here: http://www.mediafire.com/view/199datfeh7ac...ck_Chapters.xml

It should only take a minute to repackage the MP4 file into an MKV and add the chapter points. The file size will be almost identical.

Now if you play the file with VLC Media Player you will be able to see the chapter points.

I have a Sony Blu-Ray player (BDP S590) that is able to play MKV files off a USB drive, but for some reason it doesn't like the chaptered version that I created above. Usually it doesn't have any issues with chaptered MKV files that I download so I don't know what's going on... I will try it on a WDTV Live SMP player and report back.

dangum, you can skip MP4 joiner completely. You can append all the MP4s to each other directly in MKVmerge using the append button. Also, technically, it doesn't convert, it just remuxes the elementary streams. So no conversion and no quality loss. hooray!
