If anybody wants the original MPEG-2 source (approx 3.7 GB), please let me know. I have tried to author it into a DVD but haven't had much success (the MPEG-2 file has seeking errors).

have you tried reencoding? I can take a stab at fixing it.

I used TMPGEnc Authoring which re-encoded the video and that fixed the seek issue, but I want to avoid re-encoding if possible.

yes, I understand. My guess is that the headers are messed up. I haven't tried either of these, but might be worth a shot:

well, that stinks. The video might have some bigger problems. You might have to reencode. A multipass encode at a high bitrate shouldn't affect quality much at all.

so, I'm working on the DVD now (video looks good) and I was hoping to work off of some artwork. It looks like the live4evil site had CD covers up for this, but that site has been dead. Does anyone have the artwork?

The DVD is done. I should have it online tomorrow. Thanks again to Dangum for providing the video, and to berlinone for getting me those old CD covers. Here is the DVD cover, in a similar style:
